Welcome to our practice!

As a new patient, we understand that choosing an obstetrician or gynecologist can be difficult. We are glad that you have chosen us for your health care needs.

Please consider the following prior to your first visit with us:

  • Please bring a current insurance card. Your insurance carrier requires that you present it to our front desk staff prior to your visit with the physician.
  • You are responsible to make sure that any records, including referral forms for physician consults, laboratory results, or ultrasound reports, are present in the office for your visit. You may want to bring copies with you to the office on the day of your appointment.
  • Complete the new patient demographic form found in Forms on our home page. Make sure all information is complete including all phone numbers and emergency contact numbers.

In addition, please arrive 15 minutes ahead of your scheduled appointment. This allows our front desk staff to enter all of your information into our automated system and reduces the time you will wait to see your physician.

If you are unable to keep your appointment, please give at least 24 hours’ notice so that we may accommodate other patients.

Please keep in mind that the practice of obstetrics is unpredictable. We do our best to honor all appointments in a timely fashion. However, in the event of an unscheduled delivery or emergency, we may ask if you would like to reschedule your appointment at your convenience or be seen by another physician.

Medical emergencies:

Please contact 911 if your emergency is life-threatening. A physician is available at all times to handle emergent medical problems. During office hours, please contact the office to speak with your physician or nurse. After office hours or on weekends, the main number will be routed to obtain the physician on-call.

Medical questions or problems:

Our nurses are trained to answer many issues which may arise during the course of your care. Due to the volume of patient calls we receive each day, it may be necessary to leave a message on your doctor’s voice mail. We make every effort to return calls within two to three hours at most. Our physicians review most problems and may request that you come in for a visit to discuss more complicated issues. Of course, you are also welcome at any time to schedule an appointment to discuss issues directly with your doctor.

Prescription refills:

We are happy to provide routine prescription refills for established patients over the phone. Please contact your physician’s nurse and provide a phone number for your pharmacy when requesting this service. In some cases it may be necessary to have an exam or evaluation prior to receiving medications.

Medical records:

We keep all medical records strictly confidential. We follow HIPAA guidelines carefully and will only release medical information to those deemed appropriate by the patient in writing. This includes records of those under the age of 18.
